Three things: an Apple Podcasts episode link (we find the episode's audio through the show's public feed), a direct link to an .mp3 or .m4a file, or an uploaded audio file — MP3, M4A, WAV or OGG up to 200 MB.
Episodes up to about 3 hours work well. Transcription takes a few minutes for a long episode — the page keeps checking and shows the result the moment it's ready, so keep the tab open.
Not directly — Spotify doesn't expose episode audio to other apps. Most shows are also on Apple Podcasts, so search the show there and paste that link. For Spotify-exclusive shows, upload the audio file if you have it.
